Mid-Michigan Chiropractic Center (“we,” “us,” or “our”) is committed to protecting the privacy and security of

confirmations, and office notifications. Message frequency varies based on your appointment schedule.
Message and data rates may apply. You may opt out at any time by replying STOP. Reply HELP for
assistance. Consent to receive text messages is not a condition of receiving treatment. Your mobile phone
number will not be shared with third parties for marketing purposes.

If you provide consent, we may send appointment reminders and office communications to your email
address. You may opt out of email communications at any time by contacting our office or following the
unsubscribe instructions in any email we send.

Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA).
We maintain physical, electronic, and procedural safeguards to protect your protected health information. We

HIPAA obligations is available at our office upon request.

information against unauthorized access, alteration, disclosure, or destruction. Patient records are storedusing encrypted systems and access is restricted to authorized personnel only. However, no method of
electronic storage or transmission is 100% secure, and we cannot guarantee absolute security.

insurance companies for claims processing; other healthcare providers involved in your care (with your
