Terms of Service - SMS Appointment Reminders
Program Description
​Mid-Michigan Chiropractic Center offers an SMS appointment reminder service to patients who opt in. By consenting to receive text messages on our patient intake form, you agree to receive automated appointment reminder messages at the cell phone number you provided.
​Message Content and Frequency
Messages will include appointment date, time, and provider information for your upcoming visits. Message frequency varies depending on your appointment schedule. Typically, you will receive a reminder the day before your appointment and/or one hour before your appointment.

Costs
There is no charge from Mid-Michigan Chiropractic Center for receiving appointment reminder text messages. However, message and data rates from your wireless carrier may apply. Please contact your carrier for details about your messaging plan.

Opt-In
You opt in to this service by checking the "I consent to text appointment reminders" checkbox on the patient intake form completed at our office. Consent is not required as a condition of receiving chiropractic care.

Opt-Out
You may opt out of receiving text messages at any time by replying STOP to any message you receive from us. You may also opt out by contacting our office directly at (989) 224-8688 or by notifying our front desk staff. After opting out, you will receive a confirmation message and no further texts will be sent.
